Underwriter: The insurance company that assumes responsibility for the risk, issues insurance policies and receives premiums.
Usual, Customary and Reasonable (UCR) or Covered Expenses: An amount customarily charged for or covered for similar services and supplies which are medically necessary, recommended by a doctor, or required for treatment.
Waiting Period: A period of time when you are not covered by insurance for a particular problem or a particular time frame.
