Optional Accidental Death & Dismemberment Insurance Print Email Text size

Accidental death and dismemberment insurance (AD&D) protects you and your family (depending on your election) in case of a death or loss of a bodily function due to a covered accident.

Employees may elect coverage for themselves or for themselves and their eligible dependents under the family plan.  For this plan, the term "eligible dependent" covers a spouse (under age 70) and children, including step, foster, and legally adopted children (until age 25, regardless of student status).

Employee coverage amounts are available from $10,000 to $500,000 (in multiples of $10,000).  The maximum available for employees earning less than $15,000 per year is $150,000.  If a coverage amount in excess of $150,000 is elected, that amount may not be more than 10 times the employee's base annual salary.

Employee and family

 $0.04 per $1,000 of coverage

If you are age 70 or older at the time you sustain injuries in a covered accident, your benefit reduces to 45% of the elected benefit amount; at age 75, to 35%; at age 80, to 15%; at age 85, to 10%.  Coverage for your spouse or domestic partner ends once he or she reaches age 70.

Conversion/Portability

Supplemental life insurance can also be converted or ported within 31 days from when coverage ends.  You must contact the benefits office in order to initiate this process.  View the following for more information: Life Insurance Conversion/Portability
