Optional Spouse and Domestic Partner Life Insurance Print Text size

A spouse or domestic partner must be a U.S. resident to be insured under the spouse life insurance plan.  

The spouse/partner plan offers life insurance from $5,000 to $100,000 (in $5,000 increments).

A new employee (or an employee who is newly benefits eligible) may elect up to $50,000 without the completion of the evidence of insurability form.  

If you are an existing employee who wishes to add or increase your coverage, any increase of more than $5,000 per year at open enrollment or any amount of coverage over $50,000 are subject to approval of the evidence of insurability 

The cost of spouse life coverage is based on the employee's age:



Less than 30


30 to 34


35 to 39


40 to 44


45 to 49


50 to 54


55 to 59


60 to 64


65 to 69


70 to 74


75 to 79


80 to 84


85 and older



The cost noted above is per $1,000 of coverage.  Coverage is subject to the Age Reduction Schedule. 


So, for example, if you are age 45 and wanted $25,000 of coverage for your spouse, the cost is calculated as follows:

  • $25,000 / $1,000 = 25
  • 25 x $.080 (the rate) = $2.00 per month (split between your two paychecks per month)

Change in Election

If you add new coverage or increase your coverage during benefits open enrollment more than $5,000, you will need to complete the   evidence of insurability form and return to Human Resources (MS-92).  

If you currently have coverage and your new coverage is $5,000 more and does not exceed $50,000, no evidence of insurability form is required.


Supplemental life insurance for dependents can also be converted or ported within 31 days from when coverage ends.  You must contact the benefits office in order to initiate this process.  View the following for more information: Life Insurance Conversion/Portability
